Honorable Hospital Director and Honorable Chairman of the Department of Surgery,

As Chairperson of the General Surgery Residents’ Screening and Evaluation Committee, with concurrence of the Assistant Chairpersons for Training and Research, Drs Adrian Yu and Edgardo Penserga respectively, I am recommending for graduation the following who have satisfactorily completed the General Surgery Residency Training Program of the Department of Surgery of Ospital ng Maynila Medical Center.

By the authority vested on me as Chairperson of the Department of Surgery, I accept the recommendation of the General Surgery Residents’ Screening and Evaluation Committee and I in turn present the graduates to the Honorable Hospital Director of Ospital ng Maynila Medical Center for recognition and acceptance. In addition, I like to present to the Honorable Hospital Director for approval the granting of visiting consultancy privilege to the 4 graduates for the year 2004.
